I always viewed the "hidden prologue cutscene" as scrapped quicktime gameplay converted to a short cutscene!
Say if it were a quicktime sequence, you must complete two parts so the car will start up and you can begin driving them towards an exit for Willamette (because they need gas), then inside the town they have a second quicktime event where you hide the childs eyes from the zombie schoolbus or fail it and crash them into the gas station, if you succeed you cue a cutscene fixing the girls hat and the driver notices the helicoptor above, then nearly driving into the gas station which explodes anyway, right afterwards the driver sees Franks camera flashing, now the two are driving hopelessly towards the mall parking lot with Frank ahead and they tragically crash through the entrance plaza barricade because Franks metal briefcase falls into their windsheild (always wanted to retrieve that) everyone panics inside when zombies enter their mall, eating the driver while her daughter screams (fade to black)

That would make some interesting variables to the intro. Frank is taking pictures in the chopper like normal and it shows the car driving right into the gas station blowing it up if the player only completes the first quicktime sequence getting into Willamette, but not the second one, so everything stays the same as the actual game except for the car being added.
If you complete all the quicktime sequences a bit of a different cutscene happens when your taking helicoptor photos just before the gas station part, where now he gets some great shots of the car driving up and then swerving away from the explosion so he tells the pilot to bring them ahead of the car so he can get a better angle of the reckless drivers face (on the way to the mall) thats when the military choppers make his pilot swerve causing the breifcase to fall out into their windshield, showing him react to the car crash caused from it. So now feeling guilty he wants to help them fast, rushing the pilot to the roof saying to come back there in 3 days and Frank jumps out early, hobbling.
Maybe the aerial photography part was just showing part of the map that those two shouldve had to drive on to go get the gas they so desperately needed.
Frank knowing he caused their crash from dropping his briefcase has the first objective arrow already on the roof to go help, also Carlito is not there if the quicktime events were completed. Frank hobbles through the security room without any footage to find in there of the parking lot zombie kill. Now cue a cutscene of Frank walking out to Brad who is shouting to everyone below to quickly make it upstairs, yet only the little girl is seen escaping the car to the stairs, show the survivor with the shotgun die in the action and Cliff Hudson snapping a zombies neck and escaping into a vent like he uses in Crislips, Frank says he needs to help the driver (shes dead meat). Thats when the real gameplay starts and should highlight the dropped shotgun in the first room and make it necessary to shoot the windsheild out to grab the breifcase from the car crashed into the entrance door. Escape upstairs and Brad takes the breifcase/laptop from Frank in the next cutscene just to lose it to Carlito in the food court shootout. The girl is seen crying in the background when Brad asks if anyone else made it out.
This would be alternate to the poodle lady causing the zombies to enter the barricade. Also completing the quicktime events saving the little girl could have been made vital to the acheivement for rescuing all survivors, and causing a better PP bonus/level up to start off with than the all photo taking intro and cutscenes. A playthrough where the two do not make it past the Willamette gas station would be easier for the photograph all survivors achievement, but still possible by photographing the entrance plaza group corpses before returning with the breifcase to the security room. Also a saved photo of the young girl in the security room allows you to recruit Cliff Hudson after defeating him, he will be near death and can only hold wine or bladed weapons, his captives cannot be recruited while he is armed. Cliff and his granddaughter should both be saved for the all survivors achievement.
So this is how it plays out:
An alternative start to the game would begin with "hidden cutscene (gameplay)" outside of Willamette and depending on how far you get those two survivors, will affect the start of the Frank West cutscene and its contents. After the tractor trailer crash that stops the car engine the instrumental music plays and the games pauses with a text box explaining how to complete a quicktime event/sequence. (When you see a button or combination pop up in the center of the screen use your controller to match the pictured inputs!) The first sequence would be press X to kick the zombie back "get the hell away" and then when the camera angle shows the zombie rising you must hold Lt, B and flutter Rt to turn the key starting the car.
From here you get to drive a half mile which has a few obstacles, hordes and zombies eating corpses to avoid (keeping the car from breaking) before the driver says she sees the exit to Willamette with gas/food silencing the instrumental music. You drive close enough to that off ramp to trigger a cutscene showing them driving up through town with one quicktime event, press X to cover the girls eyes from peeking at the playground and schoolbus covered with zombies or dont, crashing into the gas station. If you covered the girls eyes the mother sees Frank taking pictures above, now shes confused and the gas begins to run out so she starts driving floored towards the mall to find a way in the back. Before they get to circle the parking lot the briefcase falls into the windsheild knocking the driver unconcious and sending the car careening into entrance plaza. The survivor group inside (now including Cliff Hudson) is startled and Madonna the dog runs out into the parking lot while a zombie begins to eat the totally unconsious driver causing the young girl to scream!
The title fades in after the girls scream begins.
